• I, the person whose details appear within this application, wish to apply to become a member of the Myrtleford Golf Club Incorporated in the category as selected.
• I advise that I support the purpose of the Association and that in the event my membership application is accepted by the Association, I agree to be bound by the Rules of the Association that are in place whilst I remain a member.
• I confirm that the two current financial members of the Myrtleford Golf Club Inc., whom I have listed as providing nomination and seconding support of my application for membership, are aware of my intention to apply, and have provide me with both written and verbal acknowledgement of support for my application for the category that I have hereby selected.
